    List ALL staff members authorized to have access to view patient information from the Connect Portal. Please provide unique email addresses for each staff member. Level of Access: Staff User Types: No Read; Create Pending; Full Access/Create Orders

    • If you are an ordering provider, you may use the form below to authorize designated staff to order on your behalf—without your review. This optimizes your order workflow
    • You can add and remove staff from the designation list at any time.
    • Our system will periodically require you to review and confirm your designation list. • You cannot designate another ordering provider to order on your behalf—staff only.

    Staff member accounts have 3 access levels 1. Full Order, 2. Pending Order, 3. No order access (read only Provider Signature authorizes the selection made above.
