    First Time Clients:

    After this Form is filled out and signed, Hannah will get back to you via EMAIL and/or TEXT within 48hrs. to schedule an in person consultation. This will allow her to review this form in its entireity. The in person consultation will be about 20-30mins, and will go over this form, any question or concerns you may have, color match, review length, hair and scalp health, review homecare regimen, maintenance appointments, and lastly pricing.

    Pricing is determined per client, there is not a one size fits all. Pricing inlcudes the following:

        *Purchase of Hair

        *Custom Color Prep on Extension Hair

        *Custom Color to your Hair

        *Extension Install

        *Custom Blending Haircut 

        *Home Care Hair Guide how to maintain your new hair to optimize on extension            hair  "life span".

        * Swag Bag that includes Scrunchie, Travel Size Extension Hair Brush, and                     Full Retail Products that are essential for home care regimen. 

     

    IN CASE OF RESCHEDULING:

    We understand life happens, and we want to be resecpful of your time and ours. In the event that you need to reschedule your appointment we ask that you do so 72hrs. in advance. Please understand that our stylist books out 4-6 weeks in advance. We recommend rescheduling immediately.

    Keeping in mind that that overall health and integrity of my client's hair is top priority. If Hannah does not feel that your hair or scalp is in proper condition to receive requested services, Hannah has the right to either suggest an alternative or refuse to perform services altogether. If at any point during the service, Hannah feels that the integrity of the hair or scalp is being compromised, Hannah, will stop the service and discuss other options. in addition, if AT HOME CARE is not followed through along with home care guidelines that result in adverse reactions to your hair, scalp or extension hair that have caused damage, Hannah reserves the right to refuse reinstall or maintenance appointments. IF at any time the client is rude, disrespectful to Hannah or any at Hairchaos Studio the client may be asked leave and not return.

    We strive to give every extension client the luxury service they are investing in and leave feeling confident and excited for their new look and dream their hair. In addition, we also understand that a certain level of expectation is required. However, if you are needing an adjustment due to a malfunction, bead slippage, we would be more than happy to fix it within 7-10 days after service. If for any reason, there after issues continue and Hannah has not been contacted Hannah or Hairchaos Studio is no longer liable.

    Please arrive at ALL appointments with CLEAN, DRY HAIR. Hair must be 100% dry prior to service, free of sticky hair products. Do not wash your hair for at least 48hrs. after installation.

    The Client understands that they will be responsible for the proper care of natural, and extension hair to ensure best possible results. Hannah will provide the client with all home care instructions. Hannah will NOT be held liable for any damage, issues resulting from lack of proper at home care by the client. If extensions are not cared for properly matting, slippage, discoloration, and/or detachment may occur. It is imperative that to acknowledge any issues immediately, and to contact Hannah at Hairchaos Studio. If any issues continue to occur, then Hannah will provide you with an alternative to ensure the integrity of your hair. Removal or maintenance may be required.

    Maintenace will be necessary as the natural hair grows, and it is advised to these scheduled appointments to take place every 4-9weeks for this method of extensions. All maintenance appointments sh0uld be pre-booked before leaving the salon. The client understands that its necessary to keep up with maintenance appointments. In addition, if Hannah is not made aware of any issues with extensions from lack of home care regimen before upcoming appointments an additional fee may be required due to replacement of damaged wefts, or extended appointment timing.
