I agree to a cursory background check run through Truthfinder.com. Applicants with a history of animal abuse or neglect and/or violent/sexual crimes will be denied. Please allow up to 48 hours for processing.*
I understand that I must present a valid photo ID before I will be allowed to leave the campus with a dog. I also agree to allow the staff to make a copy of my photo ID. *
I understand that participating in the Athens Area Humane Society Doggy Day Out program does not qualify me as an official AAHS volunteer and therefor the hours cannot be used for any form of community service, academic or otherwise. In order to become a volunteer for AAHS, I must complete a separate volunteer application and complete the online volunteer orientation and training. By agreeing, I verify that all information provided above is correct and up to date. If my information changes in the future, I will notify a AAHS employee and update my information on file before taking a dog offsite. Failure to do so may permanently disqualify me from the program.*
I understand and acknowledge that AAHS uses ONLY positive reinforcement to reward good behaviors. Less desirable behaviors are never punished.*
I understand and acknowledge that AAHS will provide all equipment for the dog. I am responsible for returning to AAHS all equipment used. I agree to never use a prong collar, pinch collar, chain, electric collar, or retractable leash on an AAHS dog*
I understand and acknowledge that for the safety of myself, the animals, and others, it is highly recommended that there be no interactions with any other animals of any kind.*
I agree and acknowledge that I am not allowed to take an AAHS dog to a dog park or off leash, unless supervised in my own fenced backyard. *
