Inspired Birth Series Enrollment Agreement
Thank you for choosing Inspired Birth for your childbirth and postpartum preparation classes. The Inspired Birth Express and Inspired Birth Three Week Series are comprehensive childbirth preparation courses and will primarily focus on natural childbirth. Our postpartum preparation courses are comprehensive and up to date on current postpartum practices. We strive to provide evidence based information as well as tried and true methods for coping and support. We look forward to providing you with the information you need to achieve the birth and postpartum experience you desire.
In the case of a scheduled in-person class, the instructor reserves the right to change from in-person to virtual in the situation where low enrollment occurs or illness prevents a safe in-person class environment. A hybrid mix of in-person and virtual will usually be offered in this instance. If your class is scheduled virtually, a Zoom link will be provided to you one week before your scheduled class. For in-person classes, your class location and pertinent information will be sent to you one week prior to your class date.
Your registration is specific to the time and date selected on this form. Registration may not be shifted without instructor approval. Should illness prevent your participation in an in-person class, you must notify your instructor at least 2 hours prior to the start of class of your intent to join virtually so that Zoom arrangements can be made.
The cost for Inspired Birth series or courses are listed above. A non-refundable deposit or registration fee is required to hold your space in the courses. An invoice for the final payment will be sent to the email listed above directly after your class or the first session of your series.
Refunds will only be given in the case of extrordinary circumstances and at the discretion of your instructor. The cost of the series or class will not be prorated. A scheduling conflict on your part will not ensure a refund. Should you be unable to attend a class, arrangments can be made for a private session for an additional fee.
Please Note: A late fee of $35 will be added to your invoice if it is not finalized within 2 weeks of original invoicing.
I hereby state that I am enrolling in an Inspired Birth Class Series or stand alone one session course of my own free will. I further understand that the content of these classes is in no way intended to be construed as medical advice or as a prescription for any medical procedure. I am aware that I should seek the advice of a health-care provider to answer any health-related or pregnancy-related issues surrounding my pregnancy, labor, birth or postaprtum period.
I therefore agree that I will in no way hold the instructors of Inspired Birth, Inspired Beginnings, and Inspired Families, its owners, or its representatives responsible. Any special circumstances that could arise as a result of my experience during my pregnancy, labor, the birth of my child, or mine or my child's experience during the postpartum period are acknowledged to be outside their control. I agree that neither I nor any member of my family will make any claim or initiate any suit against any of the above-named parties now or at any time in the future.
This agreement is being signed via JotForm and you will be emailed a copy of your agreement when you sign. Please let your instructor know of any questions you might have at any point in your series or after.
