Baptism Certificate for RCIA-ACT
If your child was baptized at Holy Family Catholic Church, no baptismal certificate is necessary. If your child was baptized at another parish, contact the Church of Record (Church where baptized) and ask for a Baptismal Certificate with notations.(First Communion/Reconciliation dates). The new certificate can be dropped off at our parish office, mailed to Holy Family Catholic Church (Attn: Kelli Tull) at 6150 Pershing Ave, Fort Worth, TX 76107, emailed to ktullwaldrop@holyfamilyfw.org, and attached/submitted on this form. Please note that HFCC must keep this certificate on file for our parish records and it will not be returned to you.
