I hereby grant permission to Speaks for Trees and More and stated workshop location to use photographs and/or video taken during this event in publications, news releases, online, and other communications.*

I, legal or supervising guardian of registered youth group, authorize Speaks for Trees and More to seek, obtain, and consent to emergency medical care & treatment as deemed necessary by a medical professional.*

I hereby release, Speaks for Trees and More and stated workshop location from any and all damages, injuries, causes of action which may accrue to or be asserted by me or any registered students or youth group members, arising directly or indirectly from registered students or youth group members participation in any Leave No Trace Workshop.*

If applicable, I certify that I have collected the required Annual Permission and Health History Form from every Girl Scout attending. I will retain for the duration of the registered program or activity, releasing Speaks for Trees and More and confirmed workshop location from responsibility of distribution and collection.
