All Dynamic MPS employees,
Workplace safety is an important topic in Dynamic MPS. We strive to provide you with a safe environment to ensure the highest level of quality and productivity at work. With this in mind, when you accept an assignment from this office, we and/or the company where you are assigned may provide you with Personal Protective Equipment (PPE). This equipment is given to you for YOUR SAFETY and we ask that you use Io at all times while working, no exceptions! Some of our customers require Io to use upon entering their premises, so please note if this applies to your assignment. Comply with all security rules provided by Dynamic MPS. If you have any questions about Dynamic MPS safety rules, PPE or creating a safe work environment and how it applies to your specific assignment, please consult your Dynamic MPS representative at the time you are offered the position.
If you are injured during an assignment with Dynamic MPS, there are certain rules that must be observed. They are as follows: (1) Notify your supervisor IMMEDIATELY; (2) Ask your supervisor to contact the Dynamic MPS personnel office or your representative (if outside of regular hours) to inform us of the accident and let us know what happened and if you need immediate medical attention; (3) If you need immediate medical treatment, you should see one of the caregivers in our network. (In case the injury is life-threatening, you will be transferred to the nearest hospital emergency center); (4) Upon arriving at the medical treatment center, let them know that you work for Dynamic MPS and that the injury is work-related; (5) In accordance with this employment contract, you agree to submit to a post-accident drug test. If you fail or refuse to take a drug test after an accident, your ability to receive workers' compensation benefits will be affected or denied; (6) If you are cared for and discharged, you must report to the Dynamic MPS personnel office assigned to you, as soon as possible (no more than 24 hours after the incident), with all documentation of the treatment; (7) If you are hospitalized, you or (if you cannot) a family member may contact the Dynamic MPS personnel office or your representative the next business day to let us know; (8) You must proceed with all follow-up/treatment appointments as prescribed by the attending physician, provide follow-up care documentation to the Dynamic MPS personnel office, and report for modified task assignment if released for modified tasks. Failure to do so will affect your ability to receive Workers' Compensation Benefits; (9) If you need a specialist, your attending physician will refer you; (10) You will have to pay the bill if you receive care from someone other than a network doctor without the network's approval.
Making a false or fraudulent workers' compensation claim is a crime that can result in fines and/or imprisonment. Failure to comply with these rules will result in you PERSONALLY being liable for all bills generated by your injury.
Remember, as an assigned Dynamic MPS associate, behave safely and responsibly and be aware of potential security hazards in your environment.
