The Academy conducts background checks to ensure the safety and security of its clients, staff, and volunteers. This information will be used solely for the purpose of evaluating my eligibility to volunteer with the Academy.

Liability Waiver & Release Agreement
I, {fullLegal}, understand that by volunteering my time, services, and assistance to Caddo K9 Academy ("the Organization"), I am engaging in activities that may involve certain risks and potential hazards. In consideration for being allowed to volunteer, I, on behalf of myself, my heirs, executors, administrators, and assigns, do hereby agree as follows:

1. Assumption of Risk: I understand that volunteering with Caddo K9 Academy may involve activities around dogs, including but not limited to training, handling, and interacting with dogs. I am aware that dogs, regardless of their training or temperament, may pose inherent risks, including bites, scratches, or other injuries. I voluntarily assume all risks associated with volunteering and interacting with dogs.

2. Release and Waiver of Liability: I hereby release, discharge, and hold harmless Caddo K9 Academy, its employees, agents, representatives, and all other persons or entities acting on its behalf, from any and all claims, liabilities, demands, actions, causes of action, costs, and expenses (including attorney's fees) arising out of or in connection with my volunteer activities, including any injury, damage, or harm I may suffer, whether such injury, damage, or harm is caused by negligence or any other act or omission.

3. Medical Authorization and Treatment: In the event of an emergency or if I am unable to make medical decisions for myself, I authorize Caddo K9 Academy to seek and obtain medical treatment or care on my behalf. I understand that I am responsible for any medical expenses incurred.

4. Confidentiality: I understand that during my volunteer work, I may have access to confidential and proprietary information about Caddo K9 Academy, its clients, and its operations. I agree to maintain the strictest confidentiality regarding all such information.

5. Photo/Video Release: I grant permission to Caddo K9 Academy to use any photographs or videos taken of me during my volunteer activities for promotional or informational purposes.

6. Compliance with Policies: I agree to abide by all policies, guidelines, and instructions provided by Caddo K9 Academy in relation to my volunteer activities.

7. Term: This waiver and release agreement shall remain in effect for the duration of my volunteer service with Caddo K9 Academy.

8. Parental Consent: If the Volunteer is under the age of 18, the undersigned parent or legal guardian hereby consents to the Volunteer's participation in the activities and agrees to be bound by the terms of this Agreement on behalf of the Volunteer.

I have read this Volunteer Liability Waiver and Release Agreement, and I fully understand its terms. I am signing it voluntarily and with full knowledge of its significance.
