CCA Baptism Interest Form Logo
  • Baptism Interest Form

Please complete this form if you are interested in baptism for yourself and/or your child(ren). Baptism dates for 2026: January 11, February 1, April 26, May 31, June 28, August 30 and October 25.
  • Baptism of Children and Membership

Christ Church holds the sacrament of baptism as a public and communal act. Because of our commitment to the spiritual formation of our children in the context of Christian community, we only baptize children of a household in which at least one parent or guardian is an active member of Christ Church. If you are interested in exploring church membership, please complete the rest of this form and we will be in touch!
  • Confirmation/Reception/Reaffirmation

Christ Church affirms baptisms celebrated in another Christian church as valid and therefore does not practice re-baptism. We practice Confirmation, Reception, and/or Reaffirmation or Renewal of Baptismal Vows for those who have already been baptized in another Christian church but desire to publicly affirm their faith. We'd love to explore what God is doing in your life and plan the most appropriate time and place to celebrate this you. Please complete the rest of the form and we will be in touch with more information.

  • Godparents

We encourage you to think of the role of a godparent as spiritual. As you select godparents, we encourage you to consider who you might like to have as a committed spiritual friend/guide for the candidate. A godparent should be a baptized Christian individual or couple who commits to pray for and support the child in their life in Christ by modeling Christian discipleship. Godparents make promises in their own names and also take vows on behalf of their candidates as part of the baptism service. You are encouraged to prayerfully select people who might serve as a model of faith for the candidate. Godparents could be church friends, life group members, family members, or older spiritual guides.
