MOBILE MISSIONS, LLC PHOTO & VIDEO RELEASE FORM

For the consideration of participation in any activity, event, party, or service ("Activity") organized or provided by Mobile Missions, LLC ("Company"), either now or in the future, and other good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, I hereby grant Mobile Missions, LLC the absolute, irrevocable right and unrestricted permission to use, modify, and publish photographs, videos, and/or voice recordings of me, taken on the date of any such Activity I participate in, for any lawful purpose, including but not limited to: promotional materials, website content, advertisements, and any other related endeavors. This includes the use of any printed or electronic media.

I understand and agree that the Company has the right to alter, crop, or treat the photographs, videos, or voice recordings at its discretion.

I understand and agree that I will not receive any form of compensation for the use of these photographs, videos, and voice recordings beyond the aforementioned consideration.

I hereby release and discharge the Company from any and all claims and demands arising out of or in connection with the use of the photographs, videos, voice recordings, including without limitation any and all claims for invasion of privacy, right of publicity, defamation, copyright infringement, or any other cause of action arising out of the production, distribution, broadcast, or exhibition of the aforementioned.

Irrevocability: This grant of rights and release is irrevocable and shall remain in effect indefinitely.

Jurisdiction, Venue, and Governing Law: This release is intended to be interpreted and enforced in accordance with the laws of the State of Minnesota, without regard to its conflict of laws principles. Any legal disputes arising out of this release will, to the extent legally permissible, be resolved in the courts of the State of Minnesota.

Integration Clause: This release constitutes the entire agreement between the parties and supersedes all prior negotiations, understandings, and agreements between the parties. No representations, promises, or inducements not included in this release shall be binding upon any party. 

I hereby represent that I am either 18 years of age or older and am signing on my behalf, or, if signing on behalf of a minor younger than 18 years of age, I assert that I am the parent or legal guardian of said minor and have the legal authority to grant the rights and permissions contained in this release on the minor's behalf.

By e-signing below, I acknowledge that I have read the foregoing, fully understand its contents, and confirm that my e-signature is legally binding and equivalent to my handwritten signature.
