Annunciation Church & School Facilities Usage Request Form. This is the first step in requesting to use space at Annunciation Church or School. Once we receive your request, we will do the following: 1. Check the Parish or School calendar for any conflicts. 2. Get authorization from Pastor, Principal, Business Administrator or all of the above. 3. Then you will receive an email either confirming or declining your request and outlining next steps.
