TO SCHEDULE, READ CAREFULLY:
1. First, book each child's appt to ensure they are close together (submit this form multiple times)
2. Next, immediately complete the Pre-Vaccination Form for each child.
  1. In your appt confirmation email there is a link to the Pre-Vaccination Form or you can go to metropeds.com/vaccine, scroll to bottom of page.
  2. We must cancel the appt if we see that a pre-vaccination form is not completed immediately after scheduling this appt as we can't move forward without it & there is high demand for these appts.

  - Availability also fluctuates as appointments are cancelled/rescheduled.
- Weekend flu events are currently scheduled online
- We also have a limited number of appts available Monday-Friday on defined days- please call the clinic to check for appt availability.
- If you do not see an option to pick a date and time below it means we don't have any appts currently in that location. You will also not be able to submit the form. Choose a different location to check availability.
- We are not yet vaccinating parents.
- Well child check scheduled? We offer COVID/Flu to siblings in attendance when another child is scheduled for an annual Well Child exam; you are not required to schedule a separate appointment unless you wish to have them vaccinated sooner.

We have a specific number of appointments available, with no flexibility, to ensure we can offer our families the highest amount of availability possible. It is critical that you arrive on time; we ask that you expect to be turned away should you arrive outside your scheduled time as we will not have the staffing available to accommodate late arrivals & also out of respect for other families in attendance. We appreciate that schedules are busy and sometimes things happen; we will have opportunities to reschedule for a different time should you not be able to make your appointment time.
