OMAC Waiver of Liability for Participation

OMAC World Class Martial Arts and its Instructors take the safety of all participants seriously, and will take every precaution possible to ensure the safety of participants.

The risk of sustaining injuries that result from the nature of the activities can occur without fault by the participant, OMAC World Class Martial Arts, its employees/volunteers, or the facility where the activity is taking place.

By choosing to register and take part in the OMAC World Class Martial Arts program, you are accepting the risk that you may be injured. The chance of injury occurring can be reduced by carefully following instructions at all times while engaged in program activities, and by using any necessary safety equipment such as shoes, clothes, or other.

I consent to receive services from OMAC World Class Martial Arts and its Instructors, and am registering for their program voluntarily. This consent will remain in effect for the duration of the program. I understand and agree to receive the program services delivered as part of the OMAC World Class Martial Arts program I have registered in. Program activities such as recreation activities, exercise, and fitness involve certain elements of risk. Injuries may occur while participating in these activities.

I give my permission and consent to participate in the program activities hosted by OMAC World Class Martial Arts and its Instructors. I waive my legal rights against OMAC World Class Martial Arts and its Instructors for any loss, injury, illness, or damage suffered during or by reason of participating in any and all events, programs, and activities scheduled while I am in the program.
