At Valentine K9 Rescue (VK9R), we put a lot of love, care and resources into our rescues to give them a better life. We do our best to match every dog with the right family and disclose all known information about the dog no matter whether it is positive or negative. All expenses related to care of our foster dogs are covered by VK9R. VK9R retains ownership and all decision making in regards to the health and well being of the foster dog. How long will the dog be left alone at any given time?

Does anyone in the family have any known allergies to dogs?

Do you agree to contact us immediately and provide a minimum of 2 weeks notice if you are no longer able to care for this dog(s)? (We will always work with our human fosters to help get our canine fosters into new living arrangements as quick as possible.)
Do you agree to provide regular health care by a licensed veterinarian as chosen by Valentine K9 Rescue?
Do you have a regular veterinarian? If yes, please provide name and contact information. We may decide to use your veterinarianas the vet of choice if they are willing to work with us on rescue pricing.
Do you agree to provide health care and monthly heartworm prevention at the expense of VK9R to this foster dog? This may also mean transporting to and from veterinarian appointments or making the dog available for someone from the rescue to help get the dog to the vet for care if you are unable.
Do you agree to do everything possible to keep the dog safe AND agree to keep a collar with our current contact information on the dog at all times?
AGREEMENT

By signing below, you agree the information provided on this application is true and correct to the best of your knowledge. Furthermore, you agree that any falsification of information will result in an automatic disapproval of your application. If information is found to be falsified after fostering begins, you agree to return the dog(s) to us immediately and voluntarily.

If for any reason you cannot care for this dog any longer, you agree to return the dog to us voluntarily and provide us a minimum of 2 weeks to make arrangements for new housing. You are not permitted to rehome the dog for any reason.
