  • Please Note:

    This service carries a monthly fee of $40 and a per delivery charge of $0.20.

    ALL DELIVERY RECIPIENTS MUST PRESENT VALID PHOTO ID (DRIVERS LICENSE PREFERRED) AND BE PRESENT AT TIME OF DELIVERY. WE WILL NOT LEAVE AGE RESTRICTED DELIVERIES.*

    *If your client isn't present for a restricted delivery, we will leave a delivery slip asking them to contact you to arrange re-deilvery. You can then re-dispatch the order through the portal.

    If you have a mix of restricted/unrestricted, you will need to indicate which orders DO NOT require verification, or we will verify by default.

    It is only required for the following products:

    • Adult-Oriented Products
    • Alcohol Products: Client must also not present any signs of intoxication at time of delivery
    • Cannabis and Cannabis-derivative products
    • Cigarettes and Tobacco Products
    • Vaping Products

  • By Initialing this form you attest that you have answered to the best of your ability all age-restricted products that we may be delivering.

    You also agree and are aware that failed delivery attempts for restricted products will result in additional delivery charges.

  • By Signing this document, You agree that:

    • You will be contacted by a representative of Voxzu Logstics for the purpose of setting up your account.
    • Your Name, Email and Phone Number will be used for the purposes of setting up your account in our shipping platform.
    • You will receive emails from Shipday.
    • You will receive an invoice from Voxzu Corp. for the services rendered, on a weekly and monthly basis.
    • You understand that payment terms are 7 days from Date of Invoice Issue.

  • If you have subscribed to our restricted product service, you additionally agree:

    • You will receive a monthly invoice for the service.
    • You will receive an additional $0.20 charge per delivery.

  • After you submit this form, you will be directed to setup a meeting with an onboarding specialist. Please pick the best available time for you and we will contact you then!
