Application for Raffle License

In accordance with the legislation entitled "Raffles and Poker Runs Act'. 230 ILCS Par. 15/0.01 et seg. (2016) as amended, hereinafter called "Raffles Act" authorizes the governing body of any county or municipality within the state of Illinois to establish a system of licensing of raffles.

A raffle license shall be issued only to a bona fide non-profit religious, charitable, labor, fraternal, and educational or veterans' organization.

This license is valid for one raffle or for a specified number of raffles during a specified period not to exceed one year and may be suspended or revoked by the Village of Crete Corporate Authorities in the event of violation of the terms of Village of Crete Municipal Ordinance 2018-15 or of any applicable State of Illinois law.

Has the applicant, corporation, organization or any officer, director or employee thereof, or any person who will participate in the management and/or operation of any of the raffle(s), or any person having a proprietary, equitable or credit interest in said corporation or organization ever been convicted of a felony?

Has the applicant, corporation, organization or any officer, director or employee thereof, or any person who will participate in the management and/or operation of any of the raffle(s), or any person having a proprietary, equitable or credit interest in said corporation or organization ever been convicted of a violation of the local ordinance or the ordinance of any other municipality or a federal or state statute regulating gambling?

Is the applicant, corporation, organization or any officer, director or employee thereof, or
any person who will participate in the management and/or operation of any of the
raffle(s), or any person having a proprietary, equitable or credit interest in said
corporation or organization a professional gambler or gambling promoter or has ever
been a professional gambler or gambling promoter?

Limitations:

• The total retail value of all prizes or merchandise awarded by a licensee in a
  single raffle is $250,000.00.

• The maximum retail value of each prize awarded by a licensee in a single raffle is limited to $200,000.00

 

I do hereby swear (attest) that the statements contained in this application are true and
correct to the best of my knowledge and belief. I, as the applicant, further attest that the
organization in which I am representing is in fact a not-for-profit organization and the
conducting of this raffle will not violate any provision of the Village of Crete Municipal
Code or the State of Illinois Raffles Act. (230 ILCS 15/0.01 et seq.) I further affirm that
the organization will maintain records regarding said raffle(s) in accordance with Section
6 of the Raffles Act (230 ILCS 15/6) and shall furnish to the Village of Crete
documentation evidencing the organization’s gross receipts, expenses and net profits
from each raffle and the distribution of net proceeds itemized as required under Section 6,
within thirty (30) days of the completion of the raffle. If a license is valid for more than
thirty (30) days, the report shall be made monthly.
