DIET AND ELIMINATION:
What type of food is your dog fed (Please include brand, how much and how many times a day they are fed)
Does your dog receive chews, treats or bones? Please list their top 3 favorite
Has your dog ever growled at you or another animal/person for being around their food or trying to take something away? If yes, please explain
Is your dog fully house trained? Do they potty outside or on pee pads?
How many times a day does your dog have the opportunity to use the washroom? How many hours can they generally hold it for?
Before you leave the house and leave them alone, do you provide your dog with a potty break?

EXERCISE:
Who is normally responsible for exercising your dog?
What does your dog's exercise routine look like? On leash or off leash walks? Quick potty breaks around the block? Fetch? Backyard time?
If walks are provided, what type of collar and leash is being used? Collar examples: regular buckle collar, head halter, slip lead, body harness, pinch/prong collar, choke chain. Leash examples: 6 foot nylon leash, retractable leash, long line
Does your dog ever become reactive toward other dogs or people on walks? If so, please describe
