Indoor Action Cricket League Waiver Form

Please read before agreeing

I, the undersigned participant, wish to voluntarily participate in the Indoor Action Cricket League organized by Action Premier League in collaboration with Federation of Tamil Canadians and Markham Wesley Centre. In consideration for being allowed to participate in the league and use the facilities, I hereby acknowledge and agree to the following:

I understand and acknowledge that participating in the Indoor Action Cricket League involves inherent risks and dangers that could lead to injury, illness, or even death. These risks include, but are not limited to, slips, falls, collisions with other participants or equipment, and the use of sporting equipment.

I certify that I am physically and mentally capable of participating in this league. I acknowledge that it is my responsibility to consult with a healthcare provider to ensure that I am in proper physical condition to participate. Accepts responsibility for the decision to continue participation if suffering from injuries.

I agree to follow all the rules and regulations of the Indoor Action Cricket League and any instructions given by the league organizers and officials. I understand that failure to comply may result in my removal from the league.

I release Action Premier League, its officers, employees, agents, and volunteers from any liability, claims, demands, or causes of action for personal injury, property damage, or other loss that may arise as a result of my participation in the Indoor Action Cricket League.

I understand that Action Premier League or Federation of Tamil Canadians or Markham Wesley Centre does not provide personal accident insurance coverage for any participants, and I am encouraged to obtain my own insurance to cover any personal injuries or damages. Accepts responsibility for all medical expenses incurred in case of emergency, accident, or illness and authorizes ambulance transport to the hospital. Authorizes physicians, first-aid personnel, nurses, and dentists to perform any diagnostic, treatment, or operative procedures and x-rays. No guarantee has been given as to the results of the examination or treatment. Accepts total responsibility for all medical costs.

I grant permission for Action Premier League, Federation of Tamil Canadians, and Markham Wesley Centre to use my image or likeness in photographs, video recordings, or other media for promotional or marketing purposes related to the league.

I agree not to sue or bring any legal action against the Action Premier League, Federation of Tamil Canadians, and Markham Wesley Centre for any claims, losses, injuries, or damages covered by this waiver.

I have read and fully understand this waiver form. I voluntarily and willingly accept its terms and conditions.
