Some one will call you to set up a time to come in and pay your Spay or Neuter fee you can pay by card but it will charge a 10% fee

Owner Requirements: I will be on time for pick up. Due to space of the rescue dogs we can not keep pets after the surgery unless you pay a bording fee. It is best that you be on time.

Procedures: I understand my pet will receve all core vaccinations (FVRC AND RABIRES IF MY PET IS A CAT) (DAPP AND RABIRES IF MY PET IS A DOG). I understand that my pet will undergo anesthesis and receive surgery to remove their reproductive track(spay/neuter).

Risk Of Anesthesia: I understand that anesthesia carry inherent risk of death.

Tattoo:I understnd my pet will receive a permanent green mark to indicate that they have been Spayed/Neutered.

Spay or Neuter for cat is $80.00

Spay or Neuter for dog is $100.00 if the dog is over 50lbs it is a $30.00 added charge.

All Fees are due at the time of the appointment. Missed appointment fees will not be refunded.

Each pet must be in a crate to travel to the vet with us.

once this form is sent back in you will be contacted within 24 hours to make the appointment.
