I am over the age of 18I understand that the use of the infrared sauna involves certain risks, including but not limited to overheating, dehydration, and changes in blood pressure. I voluntarily choose to participate in sauna sessions and assume all risks associated with its use.I certify that I am in good health and do not have any medical conditions that would prevent me from safely using the infrared sauna. I acknowledge that it is my responsibility to consult with a medical professional before using the sauna if I have any concerns about my health or medical conditionsI also understand that if I have any underlying medical conditions, such as heart disease, diabetes, or high blood pressure, I should consult with a physician before using the infrared sauna. I acknowledge that I have been advised to not use the sauna if I am pregnant or have any other health concerns that may be aggravated by heat exposure.I agree to follow all posted guidelines and instructions for the proper use of the sauna facilities. I will not engage in any behavior that may damage the sauna equipment or endanger myself or others.
By signing this form I release and waive any claims, demands, actions, or causes of action against SOUL FLOAT LTD, its owners, employees, and agents, arising out of or related to my use of the infrared sauna facilities. I understand that SOUL FLOAT LTD is not responsible for any injuries or health issues that may occur during or after sauna use.

I agree to indemnify and hold harmless SOUL FLOAT LTD from any liability, loss, damage, or expense, including attorney's fees, arising out of my use of the sauna facilities.
I have read and consent to all Soul Float Ltd terms and conditions and find no reason why I an unable to safely use the infrared sauna. I confirm this by providing my signature below:
