We are pleased to welcome you to our Medication Synchronization Program. The Medication Synchronization Program is designed to refill all of your medications on the same day each month. You will no longer need to remember to call in your regular medications. We will automatically have them ready for you when you need them.
How Does Synchronization Work:
Synchronization is incredibly simple. When you're almost out of your prescriptions, our computer will let us know that it's time to refill them. We will automatically fill and mail, deliver, or alert you that medications are ready for pick-up at the pharmacy.
What Types of Medications can be filled on Synchronization:
Any medications you routinely use acording to the directions we have on the label can be placed on synchronization.
What Advantages Does Synchronization Offer:
· It’s Convenient - Fewer trips to the pharmacy.
· It’s Personalized - Pharmacist consultation regarding your medications, available as needed.
· It’s Easy - Pick up your prescriptions the same day each month.
· It's just another way we can help you improve your health by making sure your medications are taken correctly.
· A full refund will be provided if we fill a medication that you notified the pharmacy to remove from your list of synced medications.
· If needed or requested, this form and associated communications will be provided in an alternate language.
What Are Your Responsibilities for Synchronization:
You Must let us know if:
· Your directions have changed on any synchronized medication
· We have filled any of your synchronized medications too early
· If you have stopped taking any synchronized medication
· Review and re-sign the medication synchronization agreement on a yearly basis.

· To pick up medications on my assigned refill date (or be available for delivery, if applicable).
· To keep an open dialogue with my pharmacist regarding my doctor, hospital, and urgent care visits, as well as changes in my health status.
