American Heart Association Basic Life Support (BLS)

• High-quality CPR for adults, children, and infants

American Heart Association Heartsaver® CPR, AED

• Describe how high-quality CPR improves survival
• Explain the concepts of the Chain of Survival
• Recognize when someone needs CPR
• Perform high-quality CPR for an adult
• Describe how to perform CPR with help from others
• Give effective breaths using mouth-to-mouth or a mask for all age groups
• Demonstrate how to use an AED on an adult
• Perform high-quality CPR for a child*
• Demonstrate how to use an AED on a child*
• Perform high-quality CPR for an infant*
• Describe when and how to help a choking adult or child
• Demonstrate how to help a choking infant*
  
  *Child and infant modules are optional.

Our Instructor:

Classes are conducted by Moshe ("Moe") Karp, our AHA BLS Intructor, who worked as a NYC Paramedic and volunteered as the Captain of Operations and trainer for his ambulance corp for over a decade. Along with his emergency medicine tenure, he has completed is MD and MPH training at St. George's University, however has not pursued his MD licensure to practice as a physician in the US. Moe's AHA certification is aligned with Allied Training and Emergency Consulting, an AHA training site, and LifeSavers Inc., an AHA training center, Fairfield, NJ, USA.

Terms/Conditions

• Payments are non-refundable and will be collected at the time of scheduling
• Scheduling will be available for evenings and weekends; based on need
• Rescheduling will be permitted a maximum of 3 times with a minimum of 48 hours notice
• Rescheduling will be permitted a maximum of 1 times with a minimum of 24 hours notice
• Rescheduling for students with less than 24 hours notice or No-shows will be at the discretion of the trainer after conducting review for acceptable circumstance for a maximum of 1 time
• Due to changes in class materials costs, classes must be completed within 60 days of initial booking to avoid penalty fees.   
• Employer/Group rates to be determined based on number of students and location
• Small group private classes reegistered for 2 students will have surcharge of $30 (couples classes)
• Private individual class registered for 1:1 will have a surcharge of $50, and will be conducted at the discretion, location and availability of the trainer
• Hybrid courses require certified of completion for online coursework to schedule class sessions
• Hybrid courses must be scheduled within two weeks of online course completions. 
• Online courses that have extended beyond two weeks of completion will require repetition of online course work or completion of the full in-person class at the cost to the student.  
• Accessibility will be made available for students with disabilities or special needs, however, requests and arrangements must be made at the time of scheduling/booking.
• All rules and regulations of the hosting site apply to in-person class sessions to maintain the health and safety of students and trainers. 

Consent and Waiver

I, the undersigned participant or legal guardian of the participant (if under 18 years of age), hereby consent to participate in the selected course(s) on this form.

I understand and acknowledge the following:

1. I have been informed of the nature and purpose of the 4 Elements 4 Life AHA BLS, Heartsaver® First Aid, CPR, AED, and/or Bloodborne Pathogens course(s) and have had an opportunity to ask any questions.

2. I am voluntarily participating in the course and understand that it involves training in cardiopulmonary resuscitation (CPR), automated external defibrillator (AED) use, First Aid, and basic life-saving techniques.

3. I am aware that the course may involve hands-on practice, demonstrations, and assessment of life-saving skills.

4. I understand that successful completion of the course may involve passing a written and practical skills test.

5. I consent to any necessary first aid or emergency medical treatment that may be administered by qualified personnel in the event of injury, illness, or accident during the course.

6. I release the American Heart Association, 4 Elements Health & Wellness LLC, and 4 Elements Direct Primary Care LLC and its independednt contractor Moe Karp LLC, from any liability related to any injuries or accidents that may occur during the course.

7. I grant permission for the use of photographs, video, or other media taken during the course for educational or promotional purposes by 4 Elements Health & Wellness LLC, and 4 Elements Direct Primary Care LLC and its independednt contractor Moe Karp LLC. I understand this consent may be revoked in writing at any time.

8. I understand that certification in Heartsaver courses is valid for a specific period and may require renewal or recertification.

9. I understand this is an education and training course, and that there is no guarantee of my correct performance of such skills in an emergency situation, and that the outcome in such situations is fully my responsibility.

I have read and understand the information provided in this consent form, and I agree to abide by the rules and regulations set forth by the American Heart Association, 4 Elements Health & Wellness LLC and its indepenedednt contractor, Moe Karp LLC.
