Assumption of Risk
Insurance Waiver and Release I give my permission for any Odyssey Dance Academy Instructors or staff to call a doctor in the event of an emergency. I recognize the risks of injury inherent in any dance exercise program and I am participating in the Odyssey Dance Academy program upon the expressed agreement and understanding that I am waiving and releasing the staff of Odyssey Dance Academy and Instructors from any and all claims, costs, liabilities, expenses, judgments, including attorneys fees and court costs (herein collectively "claims") arising out of my participation in the instructional programs, performances, and /or rehearsal participation and any and all participation in any event or program given or sponsored by Odyssey Dance Academy or any illness or injury resulting there from. I hereby further agree to indemnify and hold harmless the staff of Odyssey Dance Academy from and against any and all such Claims except Claims proximately caused by the gross negligence or willful misconduct of the Odyssey Dance Academy. I understand that students must be picked up directly after their class and if the student leaves the studio, for any reason, Odyssey Dance Academy will not be held responsible. Students must wait inside the studio for parents to pick them up. I HAVE READ AND UNDERSTOOD THE ABOVE ASSUMPTION OF RISK. Photo/Video Release

I hereby grant Odyssey Dance Academy permission to use my likeness in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Odyssey Dance Academy and will not be returned. I hereby irrevocably authorize Odyssey Dance Academy to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Odyssey Dance Academy from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. COVID-19 Release of Liability
The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people. Odyssey Dance Academy ("ODA") has put in place preventative measures to reduce the spread of COVID-19; however, ODA cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending ODA could increase your risk of contracting COVID-19. By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I may be exposed to or infected by COVID-19 by attending ODA and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at ODA may result from the actions, omissions, or negligence of myself and others, including, but not limited to, ODA employees, volunteers, and program participants and their families. I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at ODA or participation in ODA's programs ("Claims"). On my behalf, I hereby release, covenant not to sue, discharge, and hold harmless ODA, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of ODA, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any ODA program.
