The work area will be visually inspected prior to beginning work. The supervisor will check the appropriate box when an item passes inspection. Note a brief description of any deficiency. All deficiencies will be addressed and corrected prior to beginning work.

Upon submission, this form will go to the Saftey Department and is expected weekly.
