The work area will be visually inspected prior to beginning work. The designated inspector will check the appropriate box when an item passes inspection. Note a brief description of any deficiency. Immediately notify the Superintendant of any deficiencies. Service work is defined as any abatement work under 25 linear feet and / or under 10 square feet of material removal.

Upon submission, this form will go to the Saftey Department and is expected weekly.
