This form is to completed by Trust and Kōhanga Reo Kaimahi
Please complete this form to permit Te Kōhanga Reo National Trust to run a driver check on your licence with the New Zealand Transport Agency's (NZTA), Security application - Transport Organsiation Register Online (TORO).

Before you begin

To complete this form, you will need:
- Colour photos or scans of your driver’s licence
- Know your driver’s police vetting status
- Whānau hui minutes confirming the new designated driver request.
About your privacy

The Trust is committed to ensuring that all personal information is managed appropriately and securely and strives to uphold good practice privacy standards in the collection, storage, use, disclosure and disposal of personal information as governed by the Privacy Act 2020.
- Introduction
  
  Driver's Check allows Te Kōhanga Reo National Trust (the Trust) to check your license records, including vehicle classes, any endorsements or conditions and the status of your license. This ensures that only appropriately licensed drivers are driving Kōhanga Reo vehicles.
- About Toro
  
  TORO is a service that helps transport service licence (TSL) holders make sure they only have appropriately-licensed drivers driving their company vehicles. It's a safe and secure application set up by NZ Transport Agency Waka Kotahi. Companies who hold a TSL can use TORO to check the driver licence status for their drivers. If a driver licence status changes, TORO automatically lets the company know.
- What information can TORO tell us about you?
  
  Driver Check allows the Trust access to the following:
  - Licence classes you hold.
  - Licence endorsements you hold, e.g. Passenger (P) endorsement.
  - Any conditions on your license, e.g. the requirement to wear correcting lenses when driving.
  - Status of your licence, eg current, expired, suspended, disqualified, revoked or reinstated.
  - A passenger (P), vehicle recovery (V), driving instructor (I) or testing officer (O) endorsement is due to expire.
  - We’ve issued a warning letter because the driver has 50 or more active demerit points.
  - A driver is wanted for a demerit point suspension because they have 100 or more active demerit points.

  TORO terms and conditions of use require the:
  - Trust to have the written consent of the licence holder BEFORE accessing their licence record.
  - Licence holder to retain a copy of the written consent.
  - Licence holder’s written consent on this form.
