Out Of State Direct Submission For Utah Background Checks Consent Form

The following information must be submitted for your fingerprints to be electronically submitted (direct submission) to the Bureau of Criminal Identification (BCI). If any information is missing, we cannot complete this service.

1. Signed Consent (this form)
2. Copy of Acceptable Identification
3. Completed, Dated & Signed Live Scan Authorization Form
4. One Completed Fingerprint Card

The following information must match what is on your acceptable identification and the information on the dated and signed Live Scan Authorization Form.

By providing my signature below:

• I understand this form and all information including attachments are secured using end-to-end encryption.
• I have read and understand the Utah FAST Biometric Information Privacy Policy.
• All information and attachments I am providing on this form are my personal information and NOT on behalf of any other person.
• I understand the information on my acceptable identification, Live Scan Authorization Form, and completed fingerprint card, also known as Personal Identifiable Information (PII), will only be used for this specific service.
• I understand if my fingerprints are rejected due to poor quality and/or incorrect information on the card I provide to Utah FAST, I will have to resubmit all items, including paying for this service again.
• I authorize the release of my personal information to Utah FAST and I also authorize the release of all information to the Bureau of Criminal Identification (BCI) which will be used to search records from the State of Utah and the Federal Bureau of Investigations (FBI) as determined by their specific requirements and respected policies, including the release of any records found to the authorized requesting agency listed on the Live Scan Authorization Form.
