* This field is required.HAIR EXTENSION DEPOSIT AGREEMENT Any cancellation to your appointment must be made at least 14 days prior to your scheduled appointment or your deposit will be forfeited. If your deposit is forfeited you must make a new appointment and an additional deposit may be required. Any changes to your appointment date and time must be made 7 days prior to your scheduled appointment. If there are any changes within 7 days of your scheduled appointment a $150 rescheduling fee will be added to the balance due on the day of application. We understand unforeseen circumstances may arise and management will use discretion when necessary. Ample notice ensures we have plenty of time to offer your appointment time to someone else on the waitlist. We reserve the right to require a credit card on file to schedule future appointments if you should "no show" or cancel your appointment last minute, & to charge you the 100% of the "no showed" appointment. All deposits are forfeited if you are not following our policies. Our time is very valuable and we are unable to offer your appointment time to a guest on our wait list if no notice is given. We appreciate your cooperation and understanding. If you have any questions regarding our policies, we will be happy to discuss during your consultation. After filling out this form DANIELLE WESSLEY + STUDIO MERAKI will reach out to you for an in person consultation. This appointment will be $50 and must be paid upon booking. If you decide to continue to get hair extensions this deposit will go toward your hair extension package price. A Home Care Maintenance sheet will be given on the day of application; please refer to these instructions for an exceptional experience. All sales are final, no refunds.

* This field is required.Coloring your natural hair is a separate cost. It is NOT included in the hair extension package price. $120 an hour plus product fees is my standard pricing, I custom color everything to blend to be more lived in that way you are only coming into the salon for a color every three to four months! That definitely helps keep the cost down and budget friendly. First time clients that I've never colored before may be more than your regular maintenance appointment because I will have to fine tune everything to blend seamlessly for your IBE® custom install. Clients usually book two separate appointments because they can go longer than ten weeks before they will need colored again. All sales are final.
