If you are wanting to book for multiple tattoos, please fill out a separate form for each one.

To secure a booking you will need to put down a deposit, the amount will vary depending on the size of your tattoo.

Would you like to include a free gender affirming tattoo to your appointment?

If yes, please give detail on what you have in mind. This could be, but not exclusive to, nipple restoration for anyone who has had top surgery for gender-affirming reasons, or stretch mark tattoos for trans femmes on breasts, hips and tummies.

Now for your availability, please let me know a date period of when you plan to book this in.
