Media Release Form

De Fiets- en Hondenstop occassionally uses client photos for media publicastion when sharing email newsletters, on our website, and/or on social media.

You may choose one of the following options that indicates your preference in how your pet's photo(s) may be used.

Option 1: Any/All photos in any/all publications

Option 2: Specific photos in specific publications

Option 3: Photos may be taken and only shared with the human client via email and/or text message

Option 4: No photos may be taken of my pet

• I understand that I may always change or revoke my choice to allow the use of my image by providing a written statement including my name (and/or the names of any people or pets that may have been used in media publications) including descripting factors and dates of publication, and that this statement must be made via email or postal mail only. I understand that if I choose to revoke media use permissions from De Fiets- en Hondenstop that De Fiets- en Hondenstop will remove all images from its files, email, website and social media, but that newsletters cannot be unsent, and that De Fiets- en Hondenstop is not liable for any media that has been shared after content has been released.
• I understand that this waiver is valid for the time in which I am a client (within two years of my last service received) unless a specific time frame is specified in writing via email or postal mail.
• I understand that there will be no legal descriptors (such as addresses – including street name or house number, legal names or billing information) shared with my image when used for media purposes by De Fiets- en Hondenstop.
• I understand, and hereby waive, any right to inspect or approve the images prior to media use, whether that use is known to me or unknown, and I waive any right to royalties or other compensation arising from or related to the use of the image.
