  • Compounding Training Log

    Training must be completed every 12 months.

  • Designated Person is required by USP 795.  The Designated Person oversees the training program to ensure competency of all staff that may engage in compounding activities.  The DP will monitor and observe compounding activities and take immediate corrective action if necessary, ensure that all SOPs are fully implemented, establishes, monitors, and documents procedures for handling and storage of CNSPs and/or components of CNSPs.

  • Guidelines for measuring and mixing were provided and competency was demonstrated.*
  • Guidelines for proper use of equipment and devices used to compound nonsterile preparations were provided and competency was demonstrated.*
  • The employee demonstrated understanding of the documentation of the compounding process.*
