Important Information
Visa Support Letters will cost $30 AUD exclusive of GST. Hockey Australia will begin processing the Visa Support Letter once payment has been received.

Please pay for your Visa Support Letter fee by purchasing it through the Hockey Australia Shop. Fee must be purchased seperately per athlete.

Please include the athlete name/s in the order notes at checkout.

You will need the order reference number from the purchase to complete this form.
