De Fiets- en Hondenstop Policies

I acknowledge that a copy of the Terms of Service has been provided to me by De Fiets- en Hondenstop and furthermore acknowledge that I have read and understood these terms and policies and agree to abide by them. I acknowledge and accept full responsibility for adhering to the terms indicated for services, where applicable, are provided.

I acknowledge that a copy of the Payment and Cancellation policy has been provided to me by De Fiets- en Hondenstop and furthermore acknowledge that I have read and understood these policies and that I agree to all of them. I acknowledge and accept full responsibility for payment of all services rendered to myself by De Fiets- en Hondenstop and/or its affiliates. I understand that all services rendered by De Fiets- en Hondenstop are charged to me directly and I am solely responsible for payment. I understand that agreements regarding fee schedules, charges for cancelled appointments, late fees, taxes, or processing fees are between myself and De Fiets- en Hondenstop.

I acknowledge that a copy of the Privacy and Confidentiality Policy has been provided to me and furthermore acknowledge that I have read and understood these policies and that I agree to them. I acknowledge that in order to provide services to me, De Fiets- en Hondenstop must collect, store, and use personal data about me such as my name, contact information, photo/video (which may or may not contain my race and ethnicity), and other such data that I provide. I understand my rights in relation to that data and how to contact De Fiets- en Hondenstop to exercise my rights. I acknowledge that in certain extraordinary circumstances, De Fiets- en Hondenstop is required to disclose my personal data to comply with the law and/or to report certain suspected dangerous actions or circumstances. I also understand that by providing an emergency contact I have informed my emergency contact that their information has been provided to De Fiets- en Hondenstop and that I have informed my emergency contact of their rights as enumerated in the Privacy Policy. I attest that my emergency contact has consented to the collection and storing of their personal data (name, phone number, email address) and for the use of such data by De Fiets- en Hondnestop as described in the Privacy and Confidentiality Policy. I hereby consent to the collection of my personal data in the categories defined in the Privacy and Confidentiality Policy by De Fiets- en Hondenstop for the purposes stated within said policy.
