Examination Seat Request

The Examination Seat Request Form must be received at least 60 days prior to the examination date. Prior to sitting for an examination, you must submit an application and be approved to take that examination. Please note that that all sitting fees are non-refundable, but they are transferrable to another examination sitting or person.
