In signing below I agree that WADE Strength Systems LLC. is in no way responsible for the safekeeping of my personal belongings while attending their workshops or events. 

 I am aware that there are significant risks involved in all aspects of physical training. I understand that the reaction of the heart, lungs and vascular system to exercise cannot always be predicted with accuracy. I understand that there is a risk of certain abnormal changes occurring during or following exercise which may include abnormalities of blood pressure or heart rate; chest, arm or leg discomfort; transient light-headedness or fainting; and in rare instances, heart attack, stroke or even death. These risks include, but are not limited to: falls which can result in serious injury or death, injury or death due to negligence on the part of myself or other people around me, injury or death due to improper use or failure of equipment. I am aware that any of these above mentioned risks may result in serious injury or death to myself and or other participants. I also understand that events may be physically strenuous and I voluntarily participate in them as staff, officials, spotters and loaders or as a spectator or participant in another capacity with full knowledge that there is risk of personal injury, property loss or death. I agree that neither I, my heirs, assigns or legal representatives will sue or make any other claims of any kind whatsoever against WADE Strength Systems LLC. its owners, employees, independent contractors, its members for any personal injury, property damage/loss, or wrongful death, whether caused by negligence or otherwise.

I hereby certify that I know of no medical problems that would increase my risk of illness and injury as a result of participation in an event designed by WADE Strength systems LLC. With my full understanding of the above information, I agree to assume any and all risk associated with my participation in WADE Strength systems LLC. programs, activities and events.

 I fully understand that WADE Strength Systems LLC. events are strenuous and I choose to participate voluntarily. I accept all responsibility for my health and any results, injury or mishaps that may affect my well- being or health in any way. I waive any claims, demands, causes of action or any claims for relief whatsoever against, and release WADE Strength Systems LLC. (as well as any of its owners, employees, or other authorized agents, including independent contractors) from any and all liability, claims and/or causes of action that I may have for injuries or other damages, arising out of participation in WADE Strength Systems LLC. activities, including, but not limited to the powerlifting events at various locations over multiple days.

 I have fully read and fully understand the foregoing assumption of risk and release of liability and I understand that by my signature below this waiver obligates me to indemnify the parties named for any liability for injury or death of any person and damage to property caused by my negligent or intentional act or omission. I understand that by checking the box below I am waiving valuable legal rights.

In consideration of my participation in the WADE Strength Systems LLC event (powerlifting competition) as a competitor, as a loader/spotter, as a referee, as a platform worker, or in any other capacity, I intend to be legally bound, for not only myself but also for my heirs, executors, representatives, agents, successors, assigns, and administrators.

By signing this Release of Claims and Liability, I willingly assume full responsibility for any and all risks that I am exposing myself to as a result of my participation in programs/events and accept full responsibility for any injury or death that may result from participation in any activity or powerlifting event. 

I waive, release, and forever discharge WADE Strength Systems LLC, the meet directors(s), the loaders and spotters, the referees, all meet personnel, venue/gym owners administrators or agents, Powerlifting America and all Powerlifting America administrative personnel, agents, independent contractors, and employees associated with this competition, from any and all claims, demands, damages, costs, expenses, loss of services, actions, and causes of action, that I, my heirs, personal representatives, or assignee's, may have against WADE Strengths Systems LLC and the aforementioned parties for all injuries and damages, known or unknown, that I may incur as a result of my participation and/or involvement in the above-described event or by my use of the facility in which this competition is held.

 

 

 

 

 I have carefully read this Agreement and fully understand its contents. I am aware that this is a release and waiver of liability and sign it knowingly, voluntarily and of my own free will.
