Use this form when requesting an excused admit for an absence or tardy to school. Once the form and the appropriate documentation is received the by the school, the absence and/or tardy will be determined as "Permissible" or "Unsatisfactory" as per MDCPS Attendance policies and you will be notified of the determination.
Students have 72 hours upon their return to school to receive an admit. If the admit is NOT processed within 72 hours, the absence will be marked unsatisfactory.
PERMISSIBLE absences, tardies, and/or early sign-out include:
- Student illness: Students missing 3 or more consecutive days of school due to illness or injury are required to provide a written statement from a health care provider. The written statement must include all days the student has been absent from school as well as the reason for the absences. If a student is continually sick and repeatedly absent from school due to a specific medical condition, he or she must be under the supervision of a health care provider to receive excused absences from school.
- Medical appointment: if a student is absent or signed out early from school due to a medical appointment, a written statement from a health care provider indicating the date and time of the appointment, must be submitted to the principal.
- Death in the family
- Observance of a religious holiday or service when it is mandated for all members of a faith that such a holiday or service must be observed.
- School-sponsored event.
- Subpoena by law enforcement agency or mandatory court appearance.
UNSATISFACTORY absences, tardies, and/or early sign-outs include:
- Vacations, personal services, local non-school event, program or sporting activity.
- Elder students providing day care services for siblings or other family members
- Illness of others
- Non-compliance with immunization requirements (unless lawfully exempted)
- Car trouble
