Client services and chart information are confidential. Written authorization is required from you to release any information.
Depoit of $100 is non refundable
If client cancels an appointment booked no refund are given
If staying in a recovery home, check with them before booking to make sure that outside therapist are allowed
Treatments are NON-REFUNDABLE, they can be rescheduled to another date if anything occurs with your original surgery date

I understand Sirena 360 Massage does not charge travel fees and would like to keep it this way with your help to secure a paid parking spot for our therapists. We charge fee in the Broward county.
I understand that in the case of switching location, it’s subject to availability.
I Understand no tape, kinesiology tape, or any other tape should be applied to the skin before service Post op massage / LDM (Lymphatic Drainage Manual) or before any service booked with us, and we would refuse to provide a service over TAPE, KIDNESILOGY TAPE, OR ANY OTHER TAPE, themservice will get canceled without issuing any refund. understand the lymphatic drainage massage service is intended to move lymph fluids around the body, remove waste and toxins from the bodily tissues, reduce pain caused by liposuction, increase range of motion, improve circulation and offer a positive experience of touch.It is my choice to receive therapeutic massage as a form of therapy.
I understand we are not Nurses; we do not remove stitches, remove drainage, shower patients, or practice other special care.
I understand SIRENA 360 MASSAGE is a team of LMT (only females) that provided only mobile services to your accommodation. The general benefits of massage, possible massage contraindications, and the treatment procedure have been explained to me.
I understand that massage therapy is not a substitute for medical treatment or medications. It is recommended that I concurrently work with my doctor for any condition I may have.
I know that the massage therapist does not diagnose illness or disease, does not prescribe medications, and that spinal manipulations are not part of massage therapy.I have stated my pertinent medical conditions, and will update the massage therapist of any changes in my health status.
I have informed the massage therapist of all my known physical conditions, medical conditions, and medications, and I will keep the massage therapist updated on any changes.
I understand there shall be no liability on the practitioner’s part due to my forgetting to relay any pertinent information. If I experience any pain or discomfort during the session, I will immediately communicate that to the therapist so the treatment can be adjusted.
I understand and agree to abide by the therapist’s policies and will not hold SIRENA 360 MASSAGE responsible for any personal injury or lost property. I understand that any price can be changed without any prior notice. responsible for any personal injury or lost property.
I understand that last-minute cancellation would count as the service was done.
