The final summary of activities report allows you to report on the progress of your grant.
Reporting guidelines:
Your report should be submitted by the due date outlined in your agreement. You are welcome to start this report at any time and update it until the due date. Jotform will email you an edit link. If this report is submitted before the due date, the committee will not review it until all reports are submitted.
Please note: this report should NOT be copied from your 5-month report and should include updated/new information.
Once your report is approved, you will receive an approval email from the Staff Liaison. Please note that you may have to additional information before your report is approved.
