Important Notes

·  Dress appropriately with proper basketball shoes (no indoor or outdoor shoes)

·  Keep the gym clean.

.  No food or drinks in the gym (water and sport drinks with caps are allowed).

·  CCH organizers reserve the right to refuse any player who does not follow our rules and conditions without refund.

Parental Permission For Emergency Treatment

In the event of illness or accident, I give permission to Capital City Hoops qualified personnel to administer First Aid and/or CPR should my child require it. I also agrant permission to Capital City Hoops and its staff to contact emergency medical services (Police, Ambulance or Fire Fighters) for any emergency treatment for my child.

Release of Liability

Although the safety of all sport activities is the primary concern, indoor sport activities at Capital City Hoops may cause injuries and/or death.  I expressly assume the risk of injury, death, and/or illness arising from any cause, and agree to waive the right to pursue any claim against Capital City Hoops and its staff and/or affiliates and the persons in charge.

Photo Release

Capital City Hoops does take photographs and videos of all training sessions and games for promotional uses. These photographs and videos are used, but not limited to CCH's website, social media pages and in marketing programs. I give authority to Capital City Hoops to use photographs and/or videos for marketing and communication purposes such as, but not limited to advitising, promotions, displays, website, newsletters, social media and brochures. Names of individuals will not accompany any photographs or videos used for the above noted purposes. Photographs and videos of large groups of individuals may be posted on our social media platforms without the permission of a parent and/or guardian.
