Parent/Guardian Permission:

I hereby give permission for my child to attend the Poised, Polished and Proper Young Ladies Enrichment Seminar at the Lake Charles Country Club from 10:00 a.m. to 2:00 p.m.

I understand that I am responsible for all workshop fees and payments due. All payments are non-refundable. Future payments may be made via PayPal to alwaysvivid@aol.com

All materials, activities, and meals will be provided. Each young lady will leave with the tools, resources, and confidence needed to begin developing a positive image with grace, poise, and self-assurance.

We are currently seeking sponsors to provide scholarship opportunities. A deposit is required to reserve your young lady’s space in the program.

I hereby release the sponsors, organizers, and owners of JPBranden Image Consulting from any and all liability related to participation in this event. I also understand that media personnel may be present to photograph, record, or interview participants, and I grant permission for the use and release of such materials.

In the event of an emergency, I authorize medical treatment for my child if deemed necessary. I understand that any medical expenses incurred will be my responsibility or billed directly to my insurance provider.
