I understand this event may be strenuous and adverse weather conditions may occur. Nevertheless, I assume the risk involved. In the event of an accident, I authorize Colorado Envirothon to provide emergency medical attention for me during this event. I have been assured that all reasonable care will be taken to prevent an incident; therefore, I will not hold:

North American Envirothon, Colorado Envirothon Committee, Colorado Association of Conservation Districts, Colorado State Conservation Board, NRCS, CSU, any employees, contract staff and/or volunteers of any of the listed government/non-
government organizations and any medical facility used in an emergency liable should an accident occur.

I also give my consent to the use of any photographs taken of me by officials of the Colorado and/or North American Envirothon, or their representatives, for editorial and/or promotional purposes.

I give permission for myself and/or my child to travel in the care of their school or sponsoring organization for the purpose of attending the Colorado Envirothon Competition.

Release of Liability

THIS IS A RELEASE OF LIABILITY.  BY SIGNING BELOW, I AM AGREEING TO  RELEASE COLORADO ENVIROTHON STAFF AND VOLUNTEERS, COLORADO ASSOCIATION OF CONSERVATION DISTRICTS DIRECTORS, STAFF AND MEMBERS, COLORADO STATE UNIVERSITY DIRECTORS, STAFF AND VOLUNTEERS, ALL PARTICIPATING CONSERVATION PARTNERS AND ALL SPONSORS AND SCHOLARSHIP PROVIDERS FROM LIABILITY.  

I HAVE THEREFORE BEEN ADVISED TO READ THIS DOCUMENT CAREFULLY BEFORE SIGNING IT.

Introduction: I understand and acknowledge that participation in Colorado Envirothon is a privilege. In consideration of this privilege, I hereby enter into this Release of Liability (the "Release").  I understand that the Release applies to all activities associated with my participation in the Colorado Envirothon program.

Assumption of Risk and Acknowledgement of Understanding:  I understand that during my session, I may participate in multiple classroom or hands-on activities.  All of these activities may require me to assist and depend on the assistance of other participants in my assigned group.  Although not desiring to discourage me from participating, Colorado Envirothon intends to make me aware that participation in these activities exposes me to certain risks, including, by way of example, the risk of personal injury, and exposure to adverse weather conditions.  By signing this Release, I expressly assume these risks, whether such risks are known or unknown to me. 

Release and Indemnification:  In consideration for the privilege of participating in Colorado Envirothon, I hereby release and hold harmless COLORADO ENVIROTHON STAFF AND VOLUNTEERS, COLORADO ASSOCIATION OF CONSERVATION DISTRICTS, DIRECTORS, STAFF AND MEMBERS, COLORADO STATE UNIVERSITY DIRECTORS, STAFF AND VOLUNTEERS, ALL PARTICIPATING  CONSERVATION PARTNERS AND ALL SPONSORS AND SCHOLARSHIP PROVIDERS (collectively, the "Released Parties"), from, and to discharge and waive, any and all claims, demands, losses, damages, and liabilities with respect to any and all property damage, personal injury, and/or loss of life, arising from my participation in Colorado Envirothon.  The foregoing sentence shall apply (without limitation) to all claims, demands, losses, damages, and liabilities described therein, whether known or unknown, foreseen, or unforeseen, future or contingent, except claims, demands, losses, damages, and liabilities arising out of the sole and exclusive gross negligence or willful and wanton misconduct of one or more of the Released Parties.  I covenant not to sue any of the Released Parties in connection with any of the claims, demands, losses, damages, and liabilities described above. The covenants and undertakings of this Release shall be binding upon me, my family, my heirs, next of kin, legal representatives, beneficiaries, successors, and assigns.

Miscellaneous:  In the event that any provision of this Release is determined to be invalid for any reason, such invalidity shall not affect the validity of any of the other provisions, which other provisions shall remain in full force and effect as if this Release had been executed with the invalid provision eliminated. I understand and agree that this Release is intended to be as broad and inclusive as permitted under applicable law.

I HAVE CAREFULLY READ THIS RELEASE OF LIABILITY.  I ATTEST THAT I AM OVER EIGHTEEN (18) YEARS OF AGE AND AM NOT A MINOR IN MY STATE OF RESIDENCE OR, IF I AM A MINOR IN SUCH STATE, THAT MY PARENTS OR LEGAL GUARDIAN HAVE ALSO SIGNED THIS FORM BELOW.

Electronic Media & Photograph Release

I am the parent/guardian of the above-named child. I hereby grant Colorado Envirothon, the Colorado Association of Conservation Districts (CACD), Colorado State University (CSU), and their agents the absolute right and permission to use photographic portraits, pictures, digital images or videotapes of My Child, or in which My Child may be included in whole or part, or reproductions thereof in color or otherwise for any lawful purpose whatsoever, including but not limited to use in any Colorado Envirothon publication or websites, without payment or any other consideration.

I hereby waive any right that I may have to inspect and/or approve the finished product or the copy that may be used in connection therewith, wherein My Child’s likeness appears, or the use to which it may be applied.

I hereby release, discharge, and agree to indemnify and hold harmless the CACD, CSU, and their agents from all claims, demands, and causes of action that I or My Child have or may have by reason of this authorization or use of My Child’s photographic portraits, pictures, digital images or videotapes, including any liability by virtue of any blurring, distortion, alteration, optical illusion, or use in composite form, whether intentional or otherwise, that may occur or be produced in the taking of said images or videotapes, or in processing tending towards the completion of the finished product, including publication on the internet, in brochures, or any other advertisements or promotional materials.

I represent that I am at least eighteen (18) years of age and am fully competent to sign this Release.
