This player has my authorization to engage in the activities of  Ann Raphael, aka Deltona KROCZ Basketball AAU.

I allow taking photography/videos of this camper that can be used by Ann Raphael, aka Deltona KROCZ Basketball AAU, portfolio or advertising/websites.

I authorized Ann Raphael, aka Deltona KROCZ  Basketball AAU, to apply judgment in regards to medical assistance in the event of an accident, injury, or illness if they are unable to contact the parent/guardian. I allow them to apply first aid, medical or surgical diagnosis and treatment as necessary.

I allow Ann Raphael, aka, Deltona KROCZ  Basketball AAU, to provide or administer the medication or supervise in self-administration.

I release Ann Raphael, aka Deltona KROCZ Basketball AAU, and any of her coaches, staff, and manager for any responsibility in case of accident, illness, death, or any/all injury during this player's enrollment.
