I am giving authorization for my child to engage in the sports activities provided by  Ann Raphael, aka Deltona KROCZ Basketball.

I allow taking photography/videos of my child that can be used by Ann Raphael, aka Deltona KROCZ Basketball, portfolio or advertising/websites.

I authorized Ann Raphael, aka Deltona KROCZ  Basketball, to apply judgment in regards to medical assistance in the event of an accident, injury, or illness if they are unable to contact the parent/guardian. I allow them to apply first aid, medical or surgical diagnosis and treatment as necessary.

I allow Ann Raphael, aka, Deltona KROCZ Basketball, to provide or administer the medication or supervise in self-administration.

I also release Ann Raphael, aka Deltona KROCZ Basketball and any of her coaches, staff, and manager for any responsibility in case of accident, illness, death, or any/all injury during this player's activiitiies/enrollment.
