    Your responses are accessible only to Sacred Garden Community Facilitators and Facilitators in Training, and will only be read when directly relevant to facilitation.

  • Personal Questions

    Although some emotional concerns may be benefited by participation, considerations in participating must be made on a case-by-case basis. All mental health concerns should be clearly and openly communicated with the prospective Facilitator as part of the health screening process. Please note that we are not medical or mental health practitioners and this is not medical or mental health care. Your full and open participation in the consideration process will support you in having a more positive experience. And your full honesty will cultivate a rapport full of trust, care, and respect with your facilitator. While it may not be appropriate for every Practitioner to engage with every sacrament, SGC will always work with you to find the most appropriate spiritual care.
  • We recognize that sometimes people are drawn to this work because they are wrestling with existential issues. At the same part time of SGC's careful practice involves finding out more about your experience.*
  • Consent

    For each sacrament practice we create a "container" that is held loving by our intentions and agreements. Different liturgies (with different sacraments) will have different intentions, and different agreements around welcomed behaviors. Some may require silence throughout the practice, while others will welcome sound and singing. One very significant dimension of these agreements is around touch, energy exchange, sensuality, and sexuality. All SGC sacrament practices are within a strictly Platonic container meaning non-sexual, non-romantic (your facilitator will explain more about what this means during your preparation.)
  • Do you agree to follow the guidance of your facilitator when it comes to upholding the agreements of the sacramental containers in which you practice with SGC? (ie. If the facilitator tells us this is a no touch ceremony I agree not to touch.)*

  • Again, we want to remind you that consent is an ongoing conversation, it is not a static, once and done declaration. It is a dynamic process that you have agency to direct in each and every moment. It is your responsibility to make sure your preferences around touch are explicitly clear to your facilitator before every ceremony. And if anything ever does not feel right in the moment, we encourage you to use immediacy and address it in the moment.In the event that something you find offensive or in violation of your safety happens, that you were not able to address in the moment, please take note of SGC's Saftey Resources.

  • How we review your questionnaire

    and an optional way to speed it up...

    After you submit this questionnaire, our health & safety team reviews your answers against SGC's published safety references before you sit in ceremony. Members whose review comes back clear for all sacraments may be able to skip the intake interview entirely, and in many cases we can complete review the same day you submit - including day-of-ceremony intakes.

    Most of that review is automatic and private: it happens inside SGC's own systems, and a health & safety team member personally confirms every result before you hear anything from us.

    One optional piece can make your review faster. When you describe medications, supplements, or substances in your own words, our team sometimes has to stop and manually match what you wrote against our safety references. With your consent, we can use an AI tool to suggest those matches instead, which shortens the wait - often the difference in whether a same-day review is possible.

    If you say yes, here's what that means:

    • Only the individual medication, supplement, or substance names you list are shared with the AI tool - never your name, contact information, birth date, or anything you write about your personal life.
    • The AI only makes suggestions. Every decision about your eligibility is made by a Facilitator and all questions are escalated to our health & safety team.
    • The AI provider does not train on or keep this information.


    Saying no is completely fine.

    Your answers get exactly the same careful review either way - a Facilitator does the first pass review of your HSQ by hand, which can take longer.
