    1. Features up to twenty-four (24) sessions in total.
    2. Up to four (4) sessions per month
    3. Sessions cost approx. $283.25 each.
    4. Session durations are 55 minutes in length.
    5. The cost per month is approximately $1,133.00.
    6. A payment plan is available.
    7. The total cost of the program is $6798.00.

    9. No Refunds. All sales are final.
    10. Unused program portions are transferrable.
    11. A reschedule is available at the coach's discretion.
    12. There is no rollover of sessions into future months.
    13. Sessions must be used in the current month.


    This Basic package features up to one (1) weekly session.

  • ENROLLMENT INSTRUCTIONS:

    Step 1: Fill out the client and student information below, numbered A–G. If you are the student, enter'same' or'self' in the duplicate information fields. If enrolling for another person, such as a child or other relative, please enter the person's full name, your relationship to them, and their contact information in items D–G.

    Step 2. Read to understand the 'Private Coaching Agreement' items 1–12 numbered below. By signing and dating below, you will be agreeing to the terms of this 'Private Coaching Agreement'.

    Step 3. Please sign and date the agreement, which indicates you understand and agree to the 'Private Coaching Agreement terms.

    Step 4. Final Step: Press the 'SUBMIT AGREEMENT AND PROCEED WITH PAYMENT' button. Upon filling out all the required information below and pressing the submit button, you will be taken to the payment portal, where you can provide credit card information, completing the enrollment process.

  • PRIVATE COACHING AGREEMENT

  • The parties in this agreement are the coach and the client, the student, or the client enrolling a student. Guy Monroe Lietzman, 'aka' Guy Monroe, The Charisma Coach, shall be referred to as 'Coach' or 'the Coach' in this agreement.

    1. NUMBER OF SESSIONS PER MONTH: The basic training program offers up to four (4) private coaching sessions, and the intensive training program offers up to eight (8) private coaching sessions of 50 minutes each, per month, for each enrolled student.

    2. UNUSED SESSIONS ARE FORFEIT: It is the student's responsibility to take advantage of sessions offered by the coach. The student does not need to take advantage of all eight coaching opportunities. Any unused sessions are forfeited at the end of the month and cannot be carried forward.

    3. PREPAYMENT REQUIRED: The client is the financially responsible party and agrees to prepay for training prior to the first session of each month in order to retain coaching services. The monthly prepayment will remain in effect until the client notifies the coach to discontinue coaching services.

    4. STUDENT RESPONSIBLE FOR SCHEDULING AND ATTENDANCE: The client agrees that the student is responsible for scheduling sessions and attending sessions and that any missed sessions may be rescheduled or disregarded as forfeitured solely at the discretion of the coach.

    5. STUDENT RESPONSIBLE FOR WELL-BEING: The client agrees that the student is fully responsible for their well-being and agrees to notify the coach the moment the student is experiencing any discomfort for any reason during coaching sessions or the application of suggested training methods.

    6. CLIENT AGREES TO SEEK PROFESSIONAL ADVICE: The client agrees and understands that voice coaching is not to be used in lieu of professional advice. The client takes full responsibility for and will seek professional guidance for legal, medical, financial, business, spiritual, and other matters. The client understands that all decisions in these areas are exclusively those of the client and student and acknowledges that the student's decisions and actions regarding these choices are the sole responsibility of the student.

    7. NO REFUNDS OFFERED: The client is made aware that all sales are final and no refunds are available for coaching services. If the student is unable to complete the training package, the remaining sessions can be transferred to another party if requested in writing.

    8. COACHING PACKAGE EXPIRATION: All training packages expire within 30 days of the expiration period.

    9. SCOPE OF TRAINING: I understand that “Voice Coaching” is a skill-based methodology intended to facilitate the advancement of personal, professional, and communication skills as well as confidence. The techniques provided are intended to assist the student in developing social confidence skills.

    10. APPLICATION IN LIFE: The client agrees that voice coaching is a multifaceted process that may involve all areas of the student's life, including work, finances, health, relationships, education, and recreation. The client agrees that the student is responsible for deciding the best ways to implement new ideas and strategies for success, which are exclusively the student's responsibility.

    11. COACHING IS NOT THERAPY: The client understands and is aware that Guy Monroe Lietzman is only a coach and does not offer any form of therapeutic or mental health services that require medical licensing. Voice coaching is not therapy and does not treat mental disorders as defined by the American Psychiatric Association. The client agrees that voice coaching is not a substitute for counseling, psychotherapy, mental health care, or substance abuse treatment and agrees to seek out medical assistance if necessary.

    12. RECORDS TO BE KEPT IN CONFIDENCE: The client permits the coach to keep training records of the student and is aware that all student information will be held as confidential unless the coach states otherwise, except as required by law.

    Acknowledgement

    I have reviewed this 'Private Coaching Agreement'. I understand and fully agree to the terms herein without reservation.

    I accept this agreement and consent to Guy Monroe Lietzman becoming the coach for myself or the student I am enrolling in this program until I notify Guy Monroe Lietzman to cancel services.

    The client agrees to all terms: By signing below, the client agrees with and promises to honor all terms of this agreement.
