Before a new puppy can go home, we need to get to know a potential puppy owner. Every puppy family has different needs, and this information form helps us identify who needs what information and what homes might make the best fit. Once an application is received we will contact you.
This questionnaire is designed to help Calimex Pomskies find the best homes. All adopting parties are required to complete this form prior to adoption. Calimex Pomskies reserves the right to deny any adoption based on answers to this questionnaire and/or interviews prior to adoptions!
