LSC University Park Academic Suspension Appeal
Academic Suspension: A student with a continuing history of low academic performance is placed on academic suspension when the student: 1) has been on academic warning and probation, 2) has a cumulative GPA below 2.0 and 3) has a semester GPA below 2.0. An academic suspension will result in the student being denied enrollment for one spring or fall semester. LSC-University Park students that wish to appeal their Academic Suspension and seek enrollment must submit this form.

I understand that if my appeal is approved, I will
Attend a PASS workshop to enroll
Enroll in 16, 14, or 12 week classes. No minimester courses. I will not be able to enroll into classes that have already started
Ensure that UP is my home campus and take a majority of classes offered by LSC University Park
Repeat failed courses that apply to my degree plan
Have a 2.0 semester GPA at the end of the term to show academic progress
Meet with a PASS Advisor until overall GPA is above 2.0
Provide grade report for enrollment into next semester, if enrolling before final grade posts
Log into D2L PASS Community Group for support and additional resources
