• Parent/Guardian Waivers and Consent Form

  • I, the undersigned parent/legal guardian of (child's full name), born on (child's date of birth), give permission for my child to participate in the Girl Talk Club hosted by Best Version of You Life Coach and Mentoring LLC by Coach Nifeesa Haqq.

  • I acknowledge and agree to the following:

    1. The Girl Talk Club meets weekly and includes age-appropriate discussions and activities focusing on:
  • Healthy friendships

    Setting Boundaries

    Health and wellness (including basic yoga and light exercise)

    Self-love, self-care,self-esteem, self-awareness

    Internet safety

    Teamwork and social development

  • 2. While every precaution will be taken for safety, I understand that activities such as yoga or exercise involve minimal physical risk. I release Best Version of You Life Coach and Mentoring LLC, Coach Nifeesa Haqq, and associated staff/ volunteers from liability for any injuries or losses that may occur during club meetings.

  • Behavior Agreement Clause

    I understand that respectful and appropriate behavior is expected from all participants at all times during Girl Talk Club sessions. To maintain a safe, positive, and supportive environment for all, the following Behavior Policy will be enforced:
  • What Constitute a Behavior Incident:

    A Behavior incident includes, but is not limited to:

    • Disrupting activities or refusing to follow instructions
    • Disrespectful language or actions toward peers or staff
    • Physical aggression or unsafe behavior 
    • Bullying,teasing, or exclusionary behavior
    • Damaging property or materials

    Consequences for Repeated Behavioral Incidents:

    1   First Incident: Verbal warning and redirection. The particpant will be Reminder of expectations and given a chance to adjust their behavior.

    2   Second incident: Written/email Warning center the parent or guardian including a description of the behavior and the steps taken.

    3   Third Incident:  One- day/session suspension from the program. the participant may return after the suspension of behavior improves.

    4   Fourth Incident:  Dismiss her from the program without refund due to continue disruptive behavior and failure to meet behavior expectations. By signing this waiver, I acknowledge that I have read and understand the behavioral expectations outlined above and agree to support a respectful, inclusive will result in progressive consequences as determined by Best Version of You Life Coach and Mentoring LLC.
