I understand that life coaching is a comprehensive process that may involve all areas of my life, including work, finances, health, relationships, education and recreation. I acknowledge that deciding how to have these issues and implement my choices is exclusively my own responsibility and that my signature acknowledges understanding and consent for coaching services.

I understand that life coaching does not treat mental health disorders as defined by the American Psychiatric Association's Diagnostic and Statistical Manual of Mental Disorders (DSM-5). I understand that life coaching is not a substitute for counseling, psychotherapy, psychoanalysis, mental health care, or substance abuse treatment and I will not use it in place of any form of therapy.

My coach may recommend a counselor, licensed psychotherapist, or psychologist if emotional or mental illness appears to be an issue.

COACH
Fees

My initial fee depends on the coaching/mentoring session you choose, and an additional materials fee (e.g., $25-$35) is assessed for the coaching folder of forms needed for use in coaching sessions. Dates and times of coaching sessions will be scheduled via email. I request payment in full for all scheduled sessions prior to the start of the first session. All coaching services are nonrefundable unless discussed beforehand. It is the responsibility of the client to maintain timely attendance at all coaching sessions. Requests to reschedule a session date and time must be submitted at least 24 hours in advance of the start of the session. I accept Cashapp, Apple Pay, Zelle and cash payments.
