  • Please note, from receiving your report request, reports will take 4 to 6 weeks to complete as they can take time to schedule and complete.

    If you request a date sooner than this deadline, your report will be subject to an additional rush fee. Please indicate below if you require this report sooner than 4 weeks (20 business days).

    Please note that the 20 business days does not include our mandatory shut down period over Christmas (21st December 2024 - 5th January 2025) or public holidays. The 20 business days is counted from the date of report request submission.

  • To assist us with writing your child’s NDIS report, please ensure you supply the formal “NDIS LIFE GOALS” you are submitting to the NDIS for consideration. These may be the same current long-term goals, or you may have developed new ones for the next plan.

  • Assistive Technology Recommendations

    Planning for your child’s Assistive Technology / Equipment needs over the next 12 months is very important, and all items must now be included in your child’s annual reporting to be considered in their annual NDIS plan development and funding.

    If any new Assistive Technology is required within a NDIS plan date, the whole plan will need to be submitted for a “soft plan” review, which could affect your child’s overall funding, and prolong the decision process. So, planning now is imperative!

    The NDIS has recently updated the way that all Assistive Technology is now reviewed and reported on and is now split into three categories and require different levels of reporting by your therapy team.

    1. Low Cost – any item under the value of $1,500
      Funding for these items is drawn from the Consumables Budget in your child’s plan
    2. Mid Cost – any item valued between $1,501 - $15,000
      Funding for these items is drawn from the Capital Budget in your child’s plan
    3. High Cost – any item over the value of $15,001 
      Funding for these items is drawn from the Capital Budget in your child’s plan

    *Assistive Technology (AT) is an umbrella term that includes assistive, adaptive, and rehabilitative devices for people with disabilities, while also including the process used in selecting, locating, and using them. Consider any equipment or individual adaptive support or device that your child needs to help with all daily living tasks, learning, positioning, communicating, or getting around - from a wheelchair through to a pair of supportive shoes or special spoon, as Assistive technology. 

    Please note that Mobi Kids is unable to comment on equipment we have not recommended.

  • Low Cost

    Please list any “Low-Cost” equipment item (things that are below $1,500 in value) you will require in your Child’s Consumerable Budget in this plan (e.g. Sensory clothing, supportive shoes, arm and leg wraps, etc). Please note, NO specific written evidence is required from a therapist for you to purchase an item under the value of $1,500 that support your child’s needs/ goals. You will need to keep a receipt of purchase for NDIS auditing purposes. Please speak with your therapy team about any general low-cost equipment items you would like considered in your child’s plan and list below (one per line).

  • Mid-Cost

    NDIS requires written evidence from a therapist to be included at the time of your child’s annual NDIS planning for any item valued between $1,501 - $15,000. At this planning stage reporting needs to mention the equipment category only (e.g. gait trainer, standing frame, simple wheelchair or stroller, communication device etc), but not the specific brand, model, size or accessories at this stage. An equipment trial does not need to have been completed before submission. Please discuss with your planner the need for funding in your child’s Capacity Building Budget to allow for equipment trials. Please speak with your therapy team about mid cost items you would like considered in your child’s plan and list below (one per line).

  • Once funding for this/ these item(s) are approved as a general line item in your child’s plan, you can arrange an equipment trial with a supplier through Mobi Kids (or your preferred provider) for you to purchase the assistive technology item(s). Equipment trials and fittings will be charged at your normal appointment rates.

  • High Cost

    NDIS requires an assistive technology report to be completed by a therapist to be included at the time of your child’s annual planning for any item valued over $15,001. An equipment trial must have been completed before submission, itemising the specific brand, model, size, and accessories required (E.g. power wheelchair, some high-tech communication devices, complex equipment). Please speak with your therapy team about arranging trials for all high-cost items you would like considered in your child’s plan and listed below (one per line).

    For each of these items, a full NDIS AT report will be created. A high cost AT report will take from 5 - 8 hours per high cost assistive technology piece of equipment and will be billed based on the treating therapist's discipline.
