    This agreement is a valid, legally binding contract between Dreadlocks for Dingoes LLC (alsoreferred to as DfD) and the dog owner (or custodian, collectively the “Owner”). The following is Dreadlocks for Dingoes current pet grooming release form. Please thoroughly read this document to ensure you are aware of our policies and procedures. If you have any questions or concerns regarding this agreement please do not hesitate to ask us for clarification.

    Please fill out one form for each dog so we may provide the best care possible.

  • Rabies vaccinations must be current in accordance with state law. Proof of must be submitted with this form or presented during check-in on the first day of your scheduled appointment. Please contact your pet's veterinarian to obtain these records.

    There are some policies Dreadlocks for Dingoes would like to inform you of, the owner, before grooming your pet: Sometimes grooming can expose or exacerbate a hidden pre-existing condition. Severely matted pets may have skin damage that is exposed when the coat is removed. Dreadlocks for Dingoes has years of experience removing this type of "pelt" and will do so with the utmost care; however, we cannot be responsible for any such damage including but not limited to abrasions, hematomas, or rashes that may result. Dreadlocks for Dingoes will always bring anything the groomer may find to the owner's attention. We may advise the owner to seek veterinary attention.

    If your pet has any medical problems, Dreadlocks for Dingoes asks to be made aware of them(i.e. seizures, arthritis, collapsing trachea, etc.) so that the groomer may take whatever steps necessary to keep your pet comfortable. If your pet has any history of biting, Dreadlocks forDingoes requires notification of this. The possibility of biting will not affect whether Dreadlocks for Dingoes may try to groom your pet, but it can help prevent injury. If your pet has some behavior issues, Dreadlocks for Dingoes will be happy to do what we can to help work through them. On occasion, we may add a hard to handle / overly aggressive or a rehabilitation fee.

    Aggressive Pets: Due to the risk involved to both the pet and the employees of Dreadlocks for Dingoes the cost of grooming aggressive pets will vary from situation to situation; however, costs start at $200 per hour. We do not base our decision of whether or not your pet is aggressive or hard to handle on grooming experiences at other establishments, only on how your pet behaves here. We have found that this atmosphere is very different from other places and pets tend to respond well in our care. Please keep in mind the cost for sedation at a vet will range from $200-$300 plus the cost of the groom. At Dreadlocks for Dingoes we do not sedate and our cost includes rehabilitation for your pet. We hope to change the way your pet responds to grooming, resulting in the removal of your pet from the "aggressive/hard to handle" status to the regular pricing ranges. At Dreadlocks for Dingoes we are extremely successful at accomplishing this with compassion and dedication. We hope you put your trust in us and we will continue to work with you and your pet unless we find it in the best interest of your pet to send him/her to a vet for sedated grooming. In the event of a bite that results in emergency medical care please understand that the cost of care and any subsequent medical fees are the responsibility of the pet owner.

    Credit Card Storage: Dreadlocks for Dingoes will require an additional credit card submission form be filled out at the time of check in for each registered customer utilizing any services. The information is encrypted and will be stored securely in your client file. For your protection we do not retain the paper copies of these forms and will destroy them promptly. This credit card may be processed for purchases made on the phone, deposits for reservations or scheduled appointments, paying for services when someone other than the owner retrieves the dog, in the event of a medical emergency, or any other occasion that might require remote payment including failure to give timely notice of appointment cancellation and no shows.

    Pick up: Unless prior arrangements have been made, we ask you please pick up your pet in a timely manner. Day care fees will apply at our regular half day (2-6 hours) and full day (6+hours) rate, as well as $l.00 per minute past the time of daycare closing (6:00 pm Monday -Friday, and 5:00 pm Saturday).

    Cancellations / No Shows: They happen and we understand schedules sometimes change. If you must cancel, we ask that you give us 24hr notice to avoid the forfeiture of your $50 deposit, or a $50 cancellation fee in the event that a deposit was not collected for your appointment. If you fail to contact us and do not appear for your scheduled appointment it will result in an automatic charge to your credit card on file up to the full cost of the groom. There is an additional $37 fee for disputed transactions, and checks that are returned for insufficient funds.

    Photography and Permission to Release: Owner grants Dreadlocks for Dingoes permission to use photo and/or video of owner’s dog for promotional purposes in print, media, television, Facebook or internet (DfD Web site) marketing. 

    Right to Refuse Services: Dreadlocks for Dingoes reserves the right to refuse services.

    Amendments: Owner agrees to the terms of Dreadlocks for Dingoes policies as they may be amended from time to time.

  • Veterinarian Medical Emergency Release Form

    This is a required form for all Dreadlocks for Dingoes participants receiving services. Please fillout one form for each dog. This form is necessary for our files and must be signed for a veterinarian to perform any medical procedure in the case your pet needs emergency medicalattention.

  • Although accidents are exceptionally rare, there is always some risk when working on a pet. Because of that, Dreadlocks for Dingoes would like to make you aware of our policies and procedures in the event of an incident. We promise to immediately report any incident that requires veterinary care to the owner, utilize first aid and if necessary, we will transport your pet to a veterinarian. Any veterinary bills will be the sole responsibility of the owner. Safety is our number one concern. We believe in honoring the trust you have given us by returning it to you. In the event of an emergency, we will attempt to contact the veterinarian of your choice, however, the nearest veterinary hospital will be utilized if Dreadlocks for Dingoes determine that circumstances dictate.

    First and foremost, the safety and well-being of your pet(s) is of the highest importance. Ensuring that your pet remains safe and well cared for is our first responsibility and as such we take it very seriously. We do our best to have our pet parents screen for pre-existing health conditions but some factors may be beyond our control. In the event that a medical emergency arises while a pet is at our facility or participating in a service that we provide it is imperative that we are immediately able to get them medical treatment at the closest available facility. We may need to notify the owners after we have secured a medical treatment center so as to avoid delays. Our goal is to get your pet medical attention as quickly as possible and avoid any distractions which may interfere with that process. Therefore: I understand that in the event of a medical emergency, Dreadlocks for Dingoes, at its sole discretion, deems-it necessary to provide the attention of a licensed veterinarian, I authorize Dreadlocks for Dingoes to transport and seek medical attention at the closest available veterinary facility. I further agree that I am financially responsible for any medical treatment my pet(s) receives as a result of a medical emergency while attending services provided by Dreadlocks for Dingoes. I agree that Dreadlocks for Dingoes is released from all liability related to transportation to and from the vet and Dreadlocks for Dingoes is not in anyway financially responsible for treatment given to my dog for sickness or emergency. If the costof medical expenses is going to exceed $__________________, (estimated medical expenses be determined by the attending veterinarian) I wish to be contacted immediately before furthe rtreatment is given.
